Patient Information

Please refer to Appointments / Fees for information on our appointments and fee structure.  Please identify yourself as having a pension card or health care card as this may reduce the fee you incur.  Each of our specialists has the right to determine their own fee structure.  Please contact our visiting specialists rooms for their fee structure.

Medicare will only provide a rebate where there is a valid referral.  It is the patients responsibility to ensure their referral is valid.

Medicare are no longer sending cheques to patients so please update your records with them.  You can do this online at my.gov.au, by post, in person or over the phone by calling 132 011.

Ballarat Cardiology is committed to respecting the dignity of each patient.  We will manage all personal information in accordance with Australian Privacy Principles under the Privacy Act 1988(Cth) and all relevant State legislation.
